Instituto Familiar de la Raza, Inc. is a multi-service community health and social service agency with an emphasis on serving the Chicano/Latino/Indígena community in San Francisco with a special focus on the diverse needs of the Mission District. Services include a wide range of mental health, HIV-related services, and social services, including health promotion, education, prevention, early intervention, case management, psychological and psychiatric interventions, and cultural/social and spiritual re-enforcement. The agency serves children, youth, adults, and families. It has a rich history of working collaboratively with other agencies to meet the needs of the Chicano/Latino/Indígena community and other cultural/racial communities in San Francisco.
JOB/PROGRAM DESCRIPTION:
Si a la Vida–Integrated HIV Services offers a continuum of HIV services to diverse Latino communities in San Francisco, including HIV prevention and mental health services. Our goals are to support Latinos in reducing their risk of STI and HIV infection and transmission, to improve the quality of life of those living with and at risk of HIV, and to reduce stigma related to HIV, STI, sexual orientation, and gender identity. We accomplish this through client-centered, nonjudgmental, culturally and linguistically appropriate services.
The SF Department of Public Health, through a multi-year contract, funds this position.
Essential Job Duties include but are not limited to the following:
- Conduct intakes for all mental health clients and open new client record
- Conduct comprehensive assessments of client’s health and psychosocial needs, make action plans with clients and conduct ongoing follow-up
- Maintain required documentation for case management activities
- Provide monthly client data reports and participate in quality assurance reviews biweekly
- Attend clinical supervision and required meetings and staff training
- Accompany clients to appointments as clinically necessary and assist with service access
- Facilitate a psycho-educational/support group as needed
- Work closely with psychotherapist, psychiatrist, primary care providers, case managers, and other staff to achieve treatment goals regarding substance abuse and HIV care
- Develop and maintain relationships with other providers and agencies, ensuring that clients have maximum access to community resources
- Administration of the voucher provided to clients
- Home visits as needed for clients with specific mobility limitations
REQUIRED SKILLS:
- Minimum 2 years experience providing case management or social work services
- Experience in Harm Reduction
- Must speak Spanish and English fluently (the majority of clients are Spanish-speaking)
- Experience working with issues related to HIV, immigration, and access to services in the Latino community
- Experience providing services to gay, lesbian, bisexual, and transgender individuals
- Ability to meet service goals, follow billing protocols, and contract compliance requirements
- Ability to work well independently as well as with a multi-disciplinary team
- Strong oral and written communication skills, as well as data management skills
- Ability to use clinical judgment to determine appropriate boundaries and support clients
EDUCATION, CERTIFICATION AND LICENSES
- Certificate or college coursework in Case Management, Health Education is strongly preferred
- A B.A. degree in health or social service discipline preferred
CONDITIONS OF CONTINUED EMPLOYMENT
- Must pass and maintain annual Tuberculosis Test
- Must pass Department of Justice Fingerprint screening
- Complete annual recurrent online training
COMPENSATION AND BENEFITS²
Salary Range: This is a full-time position with benefits; as a non-exempt position with an hourly range rate of $32.50 to
$33.00; Commensurate with work experience, qualifications, and education.
All full-time positions are based on a minimum of (35) thirty-five hours per week. This is a full-time exempt position at 35 hours a week. The position is eligible to participate in medical, dental, vision, and 403(b) plans, earn vacation, sick leave, paid holidays, two wellness days (birthday and hire date) and receive ongoing learning opportunities.
